Forms of ID/What to bring

For Live Scan services

The California Department of Justice (DOJ) accepts only current, valid and unexpired picture identification documents as a primary valid identification. The photo identification presented must have all of the following -

    • The personal descriptor information matches the information provided on the ‘Request for Live Scan Service’ form.
    • The expiration date has not elapsed.
    • The photograph reasonably matches the applicant’s physical appearance.

Acceptable primary forms of identification include any of the following:

    1. California Driver’s License
    2. Department of Motor Vehicles (DMV) identification card
    3. Out-of-state Driver’s License

What to bring for your appointment

    1. ‘Request for Live Scan Service’ 8016 form (3 completed copies)
    2. A current, valid and unexpired form of Government ID (Driver’s License or DMV ID card)
    3. Social Security Number (if required to be filled in on the form)

To request your own Criminal Records, download the appropriate form. A $25 processing fee is charged by Department of Justice (DOJ).

For Notary services

California recognizes the following types of identification as valid if it is current or issued within the last five years -

    1. California Driver license or ID card
    2. A US Passport
    3. Driver license or ID card issued by another state
    4. A driver's license issued by a Canadian or Mexican public agency authorized to issue drivers' licenses
    5. US Military Identification card
    6. Foreign passport stamped by the INS (USCIS)
    7. For identification of prisoners in custody, inmate ID issued by the California Department of Correction

All identification cards must contain a photo, a physical description, a signature, and a serial number. An international driver license cannot be accepted as proof of identification.

Identity of the signer may also be established by one or two credible witness or subscribing witness.
